Data relating to the patient's admission and stay in an institutional care facility

• Admission recommended
• Date of receipt (day, month, year)

• Reason for admission
• Acceptance (manner)
• Onset of symptoms - date (day, month, year) - validity for urgent admission
• Onset of symptoms - time (hour, minute) - validity for urgent admission
• Basic diagnosis - diagnosis of the basic disease that is the cause of hospitalization according to the International Statistical Classification of Diseases and Associated Health Problems as amended by the tenth revision (ICD-10)
• Hospitalized for the first time in his life for a basic diagnosis
• External cause of the basic diagnosis (applies not only to injuries), diagnosis according to ICD-10
• Other diagnoses (secondary diagnoses) according to ICD-10 (newly added separate section)
• Severity of injury according to AIS classification (conditionally mandatory)
• Date of operation (day, month, year)
• Operation time (hour, minute)
• Main operative diagnosis, diagnosis of the disease that is the main reason for performing the operation according to ICD-10
• Nosocomial infection (mandatory for all hospitalizations since 2007)
